National RTAP Incident and Injury Report Form Updated

This National RTAP sample Incident and Injury Report Form is designed to be completed in the event of an accident of incident when an injury has occurred. It is not intended to cover all situations. Like all National RTAP products, it may be altered to suit your agency’s needs in any way. We recently updated the form with additional personnel to report an injury to and additional types of treatment facilities.
